COMMUNITY & HOME BASED SERVICES

A one-to-one service provided to children ages 3 to adult, in the comfort of their own home & community, to assist them with daily living skills (ADL). Our providers are loving, caring and well trained at helping developmentally delayed children achieve independence and be a part of the community.

Our In-Home respite program serves individuals ages 3 and up. The program provides family members much needed relief. Respite Care Worker provides one-on-one support in the individual’s home and take them out into the community where they can generalize skills taught and encourage their likes and interests. This program is funded through OPWDD and Medicaid.

A Self-Direction program allows individuals with disabilities and their families to choose, design and manage Medicaid-funded services based on their individual needs and goals. Individuals create and manage a budget, hire support staff, and engage in activities that will enhance integration to live and work in the community. The individual may choose the mix of sports, services, and staff that best suits their needs.
